How Medicare calculates 2026 IRMAA

Social Security generally uses the federal tax return filed in 2025 for tax year 2024. MAGI for this purpose is adjusted gross income plus tax-exempt interest. The standard 2026 Part B premium is $202.90 per month. Higher-income beneficiaries pay an additional Part B amount and, if enrolled in prescription drug coverage, a separate Part D adjustment.

How to use the result

Select the filing status used on the relevant return and enter 2024 MAGI. For a married couple in which both spouses have Medicare, choose two beneficiaries; IRMAA applies to each person. Add your actual Part D plan premium if you want a broader annual estimate. Late-enrollment penalties, Medicare Advantage premiums, Medigap, deductibles and copayments are not included.

What if your income has fallen?

The two-year lookback can overstate current income after retirement or another major event. Social Security may make a new decision following specified life-changing events, including marriage, divorce, death of a spouse, stopping or reducing work, certain property losses, pension termination or certain employer settlements. Review Form SSA-44 and the official SSA premium rules.
